Were You Vaccinated Outside NYS? Let Us Know

Liberty, NY – Sullivan County Public Health Services is asking all Sullivan County residents who received a COVID-19 vaccination outside New York State to take 5 minutes to let them know.

“We’ve created a simple online form to enter your information and upload a copy of your vaccine card. Anything you submit will remain confidential,” explained Public Health Director Nancy McGraw. “Our goal is to have a truer picture of the percentage of County residents who are fully vaccinated against COVID-19.”

“The NYS Vaccine Tracker indicates that 60% of Sullivan County residents age 18 and older have had at least one vaccine dose. We believe that percentage is higher,” she added. “Our community has worked diligently together in our fight against COVID-19, and we appreciate all of your efforts. We are asking you to do one more thing: make sure you are counted as a Sullivan County resident who has been vaccinated.”

“Many of us use doctors and pharmacists and other healthcare providers in Pennsylvania, Florida and New Jersey, but these states don’t provide New York or Sullivan County with COVID-19 vaccination records,” noted District 2 Legislator Nadia Rajsz, who chairs the Legislature’s Health & Family Services Committee. “So we need to ask people individually, if we want our records to be accurate and up to date. Please participate – you’ll be helping us keep coronavirus at bay!”

NOTE: If you were vaccinated in New York State in another county, you do not need to fill out the form.
